Who we care for

Suncare Community Services diverse range of programs provide care, support, information and referral services to all members of the community including:

  • frail aged persons and their carers
  • people of all ages with a disability and their carers
  • those individuals and their carers requiring nursing or allied health support following release from hospital
  • individuals and carers who are in need of mental health support and education and
  • frail aged persons who live independently and care for themselves and needs assistance but has a neighbor, friend or family member who checks in on them

Eligibility criteria

Suncare’s services are provided within government funded community care programs, and each program has guidelines and regulations about who is entitled to receive which services. That is why it is very important to talk to one of our Support Facilitators/ Intake Officers to discuss what your situation is and the right support available in your area that meets your needs.

For many of our programs services your eligibility can be assessed over the phone, you don’t always need an Aged Care Assessment, especially in an emergency, and we may also be able to refer you to other important services.
